I sync from MMW to the phone using USB connection. I select the tracks and use "send to" from the contextual menu. I don't auto-sync anything.

Go into the files on MMW, delete all artwork in them, then add back only one image, the correct one you want to display. Delete the existing files from your device and re-sync. That should fix the issue.

I have had this issue consistently with files that have more than one artwork image in them. I sync from MM on Windows to an MMA on an HTC One M8 phone and have found that as long as there's only one image in the file, the artwork displays without issue.
