The library's import request window is hidden behind the frozen main window.

PS the database had been manually deleted before.

* delete the MM5 database
* start MM5

Do you mean the "Do you want to copy the database from MediaMonkey 4" window?
The window is showing before the main window is shown, so it can't overlap.
Or do you mean another window?

Yes, that window.

I followed the procedure reported, and can replicate the problem.

The upgrade is obscured by the main MM5 window, which sits on the top. ... MM5 appears to be frozen.
If you minimise and then restore MM5, you can see that the Upgrade dbox was sitting underneath.
