sync issues MMA & MMW

The file noted in the phone screenshot is not only not in my library, it is not even on my hard drive (I searched). However, it WAS on my phone, 2 instances of it. So, is Sync re-downloading everything I previously synced to the phone, even though I have unchecked everything else?
I only have 2 devices listed in the tree - one for the internal storage of the phone and one for the SD card. Both have identical settings as far as what I've checked/unchecked. I have tried syncing to the card and syncing to the internal storage with the same result of it trying to send way more files than I have requested.
The current MMW is the only instance on my computer, and only one MMA on my phone.

I haven't heard anything back yet on the log.
Once again, i appreciate your thoughts on possible solutions as it's made me go back and recheck things again. I'll keep playing with it and hope I hear back on the log soon.

Is the file found on your PC, and in your MMW Library after Sync?
