What causes bad rip of CDs

I have been getting an increase number of bad error data when ripping Cd's as I want to make sure the data is accurate?
I know it can be the CD itself or maybe the CD Rom?

Try another make / model of CD or DVD drive and you may find the number of errors have reduced!
